Eligibility - Population(s) Served: Adults 18 years old and over long-term illnesses/disabilities, requiring ongoing medical/nursing support that cannot be met at home or in a long-term care setting * on admission, may have limited physical and/or cognitive capacity due to medical complexity but are believed to have restorative potential
