Description of Services: Provides self-management information and workshops for both the general public and health professionals * all workshops are provided free of charge to participants
  • Living Healthy Northeast Program: Coordinates different workshops for the general public (people with chronic conditions and caregivers)
  • living well with chronic disease (6 weeks) (en français aussi)
  • living well with chronic pain (6 weeks)
  • living well with diabetes (6 weeks)
  • cancer thriving and surviving (6 weeks)
  • diabetes, healthy feet and you workshop (one time-2.5hrs workshop)
  • getting the most of your healthcare appointment (one time-2.0 hrs workshop - en français aussi)

    Workshops for Health Care Providers
  • choice and changes (level 1)
  • brief action planning (Level 2-recommend completing choices & changes first) program also coordinates and supports educational opportunities and mentorship for health care providers around self-management support
