Description of Services:	Provides self-management information and workshops for both the general public and health professionals * all workshops are provided free of charge to participants Living Healthy Northeast Program: Coordinates different workshops for the general public (people with chronic conditions and caregivers) living well with chronic disease (6 weeks) (en français aussi) living well with chronic pain (6 weeks) living well with diabetes (6 weeks) cancer thriving and surviving (6 weeks) diabetes, healthy feet and you workshop (one time-2.5hrs workshop) getting the most of your healthcare appointment (one time-2.0 hrs workshop - en français aussi) Workshops for Health Care Providers choice and changes (level 1) brief action planning (Level 2-recommend completing choices & changes first) program also coordinates and supports educational opportunities and mentorship for health care providers around self-management support
