Description of Services: Offers a new way of monitoring and supporting individuals with congestive heart failure (CHF) and/or chronic obstructive pulmonary disease (COPD) from their homes
  • provides individuals with equipment/kits necessary to measure vital signs and health information
  • equipment/kits are delivered and set up at the individual's home by a technician who also provides training
  • individuals submit daily health status updates
  • nurses monitor the data and take action if the numbers or the individual's responses to a list of questions indicate a need for intervention
  • provides health coaching sessions with all individuals on a weekly basis
